All services provided by Shade of Face FX are subject to the following policies.

Payment Agreement

All bookings will require either prepayment in full (online bookings) or deposit (manual bookings) to secure. Prepayments/deposits are non-refundable and non-transferable. Prepayments/deposits will be applied to invoice amount due at time of service completion.

All invoices are due upon receipt. Payment in full is expected within 7 days of invoice date. If unable to pay in full within 7 days, please contact Shade of Face FX to establish a payment plan. Failure to pay within 7 days and/or establish a payment plan will result in additional charges and fees to balance out any potential loss or burden incurred by Shade of Face FX.

Shade of Face FX accepts the following payment methods: cash, check, credit/debit card, cashapp, paypal or venmo.

Booking

All bookings must be made at least 3 days in advance. The limit for booking is 1 year (365 days) in advance of your project or event.

All bookings will require a completed contract and deposit in order to be secured. Deposit amounts are non-refundable. Deposit amount will be applied to the total amount due at time of invoice.

Pre-payment in full will be required for online bookings. At this time our online bookings are on a per day basis. If you have an extended project you may book by the day using our online system. To have your booking manually confirmed and secured with a 35% deposit please fill out the contact form on our website or email shadeoffacefx@yahoo.com.

Please read our cancellation policy prior to booking to understand and avoid any unnecessary penalty fees.

Cancellations

Please note that once you have booked Shade of Face FX it means that we have reserved time in our schedule exclusively for you. Should you need to cancel a booking the following policies apply.

Standard Cancellations | 48 Hours Notice

Cancellations will be accepted up to 48 hours prior to a scheduled booking. Cancellations made within 48 hour window are subject to the following basic penalty charges.

Basic Penalty Charges: Consultation Bookings - $15 | On Set Bookings - $50 per day booked

Late Cancellations | Less than 48 Hours Notice

Cancellations made with less than 48 hours advance notice will incur a penalty of 50% of the total booking amount for the duration of the project. To avoid excessive penalty charges please be sure to cancel your booking at least 48 hours in advance.

**In the event that Shade of Face FX is unable to perform services for any reason including but not limited to illness, accident, weather, etc., special accommodations will be made up to and including a full refund of deposits and services not provided.

Travel Expenses

Personal Vehicle Travel

Shade of Face FX is based in Milwaukee, WI and regularly services the following areas:

Wisconsin - Milwaukee, Wauwatosa, Brookfield, Waukesha, Madison | Illinois - Chicagoland area

A travel fee of $30 per day will be added to invoices for locations requiring use of a toll road and/or a drive of more than 2 hours to reach.

Overnight | Extended Stays

Projects requiring overnight and/or extended stays will require accommodations be provided by the client.

A private bedroom with on site bathroom and shower are required. If accommodations are not available on location (if filming at personal home or air bnb), a basic hotel room within 15 minutes driving distance of base location will be charged to the client for the duration of the project.

All travel arrangements must be secured prior to arrival on first day of project work.

Crediting

Shade of Face FX will be credited for all photos/media featuring makeup applications, custom created prosthetic pieces and/or custom created fabrication pieces.

Materials

Material costs will be given in an estimate following a completed consultation ($25 fee). Estimate will be sent following a completed consultation. Estimates are good for either 7 or 14 days, dependent upon closeness to the event booking. Estimate expiration date will be clearly listed on estimate. Any material costs will be due up front. A formal invoice will be sent for material costs. Project work will not begin until material costs are received.

Material costs will be determined by Shade of Face FX. Material costs will be determined using the lowest market values available, without compromising the quality and integrity of the work to be completed.

Any materials purchased will be the sole property of Shade of Face FX.

*Certain exceptions, determined by Shade of Face FX, will be made in specific cases. Clear notification will be made to the client in those specific cases.

Custom Prosthetics

All custom prosthetic inquiries are subject to a consultation ($25 fee) - either virtual or in person - which can be scheduled through the online booking system (prepayment in full) or contact form (deposit).

Estimate will be sent following a completed consultation. Estimates are good for either 7 or 14 days, dependent upon closeness to the event booking. Estimate expiration date will be clearly listed on estimate. Following clients acceptance of the estimate, any material cost will be due up front. A formal invoice will be sent for material costs. Project work will not begin until material costs are received. Once work begins progress shots will be sent. Final proofs will be sent to ensure design satisfaction before molding process begins. Once molding process begins no further alterations can be made.

Final payment amount will be due upon invoice. Payment in full is expected within 7 days of invoice date. If unable to pay in full within 7 days, please contact Shade of Face FX to establish a payment plan. Failure to pay within 7 days and/or establish a payment plan will result in additional charges and fees to balance out any potential loss or burden incurred by Shade of Face FX.

Shade of Face FX will be the sole owner of all custom sculpts and molds. Shade of Face FX may use any sculpts and molds created as a result of custom prosthetic orders for any and all future projects.

Shade of Face FX will be credited for all photos/media featuring custom created prosthetic pieces. Accepted crediting information below:

Failure to pay within 7 days and/or establish a payment plan will result in additional charges and fees to balance out any potential loss or burden incurred by Shade of Face FX.

Health & Wellness

Skin Conditions | Contagious Infections | Illnesses

All skin conditions shall be reported by the client to the makeup artist prior to application, and if need be, a sample test of makeup may be performed on the skin to test for reaction. Client(s) agree to release Annaliese Thaney (Shade of Face FX) from any liability for any skin complications due to allergic reactions.

Annaliese Thaney (Shade of Face FX) reserves the right to refuse or alter services if unsanitary conditions exist and/or contagious infections are present. Client(s) understand that Annaliese Thaney (Shade of Face FX) may alter service(s) or NOT perform services where there is evidence of contagious infections/illnesses such as pink eye (conjunctivitis), cold sores/fever blisters, etc.

Service alteration or cancellation due to allergic reactions, unsanitary conditions, contagious infections/illnesses, etc. will incur a penalty of 50% of total booking amount for duration of entire project.

Non-Defamation

Client and all parties obtaining services from Annaliese Thaney (Shade of Face FX) agree that they will not, directly or indirectly, make or ratify any defamatory comments, reviews or remarks as defined by law, in writing, orally or electronically, about Annaliese Thaney (Shade of Face FX) and their respective products and services. By receiving services, you agree to this contract stipulation whether or not this contract is signed by the party obtaining the service. Client may not review Annaliese Thaney (Shade of Face FX) poorly on social media or other review sites if such instance occurs.
